This form is used in a small claims action to request that the case be removed to County or District Court. If a defendant in Small Claims Court files a counterclaim that exceeds the jurisdictional limits of Small Claims Court, the defendant is required to request that the case be removed to County or District Court and mail a copy of this form to the plaintiff.
