Statement of Mechanics’ Lien with Notice of Intent to File

If you are a contractor, subcontractor, or other party involved with construction or repair of improvements on real property and someone owes you money, this is the form used to record your mechanics lien and to notify the owner and general contractor of your intent to file a lien if you are not paid within 10 days.
