This form is used by each party to let the court know that they provided the mandatory disclosure documents to the other party. It must be filed with the court within 40 days after the petiion was signed by the other party, the other party signed a Waiver and Acceptance of Service, or the other party was served with the Petition for Dissolution or Legal Separation and a summons.
