This form is used to provide proof of the sale of any kind of motorized vehicle such as a car, truck, or ski-do. It states that the vechicle is sold "as is." Proof of ownership is necessary to register a vehicle. If the seller is providing the financing for this vehicle, a chattel mortgage form (form 915) and promissory note (form 922GT) will also be needed.
